In order to register a child for school you will need the documentation noted below:

Please make sure to bring it with you.

Proof of Date of Birth (Must be original)

 Guardianship Agreement (Form is included in packet.)

To be used only if child resides with someone other than the parent.

 Proof of Immunization (You have thirty (30) days to submit to the school nurse)

Month, date and year of immunizations signed by doctor or clinic.

 Proof of Residency (You must provide two (2) forms of residency from A or B ):

A. Homeowner

• Mortgage statement/Deed/Closing papers - Required
   (Rockland County Clerk Recording cover sheet  - yellow page).
    AND 1 of the following below  
• Current home telephone bill, (i.e., Verizon, Optimum, etc.)
• Current gas and electric/oil bill (i.e., Orange & Rockland)
• Current water bill (i.e., United Water)
• Current cable TV/digital TV bill (i.e., Cablevision, Direct TV, etc.)
• Current broadband/internet service provider bill (i.e., Verizon, AOL, etc.)

B. Renter/Tenant

• Lease with a rent receipt (dated, signed by the landlord with address & telephone number of landlord and property address).
    OR
• Residency affidavit with a rent receipt (dated, signed by the landlord with address & telephone number of landlord and property address).
   
  AND ONE OF THE FOLLOWING
• Current home telephone bill, (i.e., Verizon, Optimum, etc.)
• Current gas and electric/oil bill (Orange & Rockland)
• Current water bill (i.e., United Water)
• Current cable TV/digital TV bill (i.e., Cablevision, Direct TV, etc.)
• Current broadband/internet service provider bill (i.e., Verizon, AOL, etc.)
• Section 8 documentation

Please note we cannot accept the following for proof of residency: driver's license, checkbook, bank statements, credit card statements, car insurance statements/cards, cellular phone bills, car notes or loan statements other than mortgage loans.

Proof of Immunizations Statement signed by a physician or clinic listing the month, day and year of the following immunizations including:

• 2 doses MMR (measles/mumps/rubella)
• 3 doses DPT(diptheria) 3 doses IPV or OPV polio vaccination if born on or after January 1, 1993
• 3 does Hepatitis B vaccine if born on or after January 1, 1998
• 1 Dose Varicella (Chicken Pox)
